Updated Special Programming Form

Public Services and the Business Office have updated the Special Programming Form. It has been added to the forms on the Intranet.

Please start using the new form for any upcoming programs that you might have. Also, please make sure that you delete any saved copies of the older versions of the form as the Business Office will not accept the old versions of the form for August programs. They will accept the old forms for July programs as many of those programs are in the planning process and performers have already been sent the old form.
