Who is responsible for filling out the Public Service Loan Forgiveness (PSLF) form?

- Page 2: Human Resources has a specific template for filling out the second page of the form. Please do not fill out this page.
Where should I send my PSLF form?
NOTE: Please indicate whether you prefer to have HUM fax your form or if you will be submitting it yourself.

- Interoffice mail to HUM (attn: Shree)
When can I expect my form to be completed?
Forms are typically completed as soon as they are received. You may have to wait up to one business day.
If I ask HUM to fax my form, will I receive confirmation?
Yes, we will email you a fax confirmation sheet along with a scanned PDF of the completed form.
Are all of my years of service counted?
Only full-time years of service count toward PSLF. For more information regarding eligibility, please visit StudentAid.gov.
My application was denied. What should I do?
Please send your form and any accompanying information to HUM so that we may investigate.
