FMLA

​The Family Medical Leave Act (FMLA) is designed to help employees balance their work and family responsibilities by allowing them to take reasonable unpaid leave for certain family and medical reasons. FMLA provides eligible employees with up to 12 weeks of unpaid, job-protected leave on a rolling-calendar basis. Company-provided paid leave will run concurrently with FMLA leave until the paid leave is exhausted. Then unpaid leave will be granted up to the maximum allowable.

Please contact HUM at 405-606-3739 as soon as you think you may need to use FMLA leave. HUM will evaluate your eligibility and, if eligible, walk you through the process for applying for the leave. This will include providing a form completed by your doctor.
