Health Screening Registration Now Open

If you have never had a health screening with MLS before, here is some information you may find helpful:

• Available to ALL employees, both full-time and part-time
• Completely confidential
• Earn 200 Wellness Points which is equivalent to $20 to spend in the Rewards Mall
• Measures blood pressure, resting heart rate, LDL, HDL, total cholesterol, triglycerides, fasting glucose and A1C. These tests are key determinants for risks associated with cardiovascular disease, stroke, and diabetes

Four options to choose from:

1. Onsite Biometric Screening Downtown 01/11/22 7:30am-10:30am-Register by 01/10/22
   • Takes about 15 minutes
   • Fasting highly recommended but not required. Not fasting will affect fasting glucose results and may affect triglycerides
   • Simple finger stick
   • Get results and discuss with Integris Health coach (all Integris Health coaches are fully vaccinated and will be wearing appropriate PPE)
   • SIGN UP HERE. SCHEDULE AVAILABILITY WILL BE EMAILED TO YOU A FEW DAYS AFTER YOU REGISTER
      
2. Onsite Biometric Screening Midwest City 01/12/22 8:00am-11:00am-Register by 01/10/22
   • Takes about 15 minutes

   • Simple finger stick
   • Get results and discuss with Integris Health coach (all Integris Health coaches are fully vaccinated and will be wearing appropriate PPE)

3. DLO Option available 01/03/22-01/31/22
   • Make an appointment online at any DLO location (SEE ATTACHED FLYER FOR INSTRUCTIONS)
   • Fasting highly recommended but not required. Not fasting will affect fasting glucose results and may affect triglycerides
   • Venipuncture (traditional blood draw)
   • Takes 5-10 minutes
   • Login to DLO website and see results within 48 hours
   • Schedule a virtual appointment to discuss results with Integris Health coach

4. Download Physician Form from Wellness Connect (this can be completed any time through 08/31/22)
   1. Take form to your healthcare provider to complete (may be subject to applicable deductible/copay)
   2. Upload completed form to Wellness Connect
