Additional Information for Biometric Screenings

The Metropolitan Library System is pleased to announce that we have partnered with Total Wellness to offer FREE on-site wellness screenings for our staff in 2023! This is a new vendor that allows us to offer more clinics and more options.

Biometric screenings often indicate health issues before they become chronic thus avoiding expensive health care costs, so we encourage everyone to take advantage of this benefit!

The base level biometric screening is paid for by the Library for all staff that wish to participate (full-time and part-time). When registering, you will select the following options:

• Exam – Annual Preventative Wellness Exam
• Lab
• A1C (optional)
• COVID Antibody Test (optional)

Total Wellness offers additional vaccinations and lab work. For these options, you will need to provide your insurance card (either your insurance through the Library or other insurance) or pay for the options. When registering, the options will indicate if an insurance card or payment is needed.

Here is some additional information about this year’s biometric screenings:

• Screenings must be completed on your own time.
• Fasting is not required for lab work.
• The lab work is done through a blood draw.
• All vaccines, including the flu shot, require an insurance card or payment.
• If you wish to receive a vaccination, you can fill out this form ahead of time or during the screening.
• If you choose to receive a vaccination requiring a series of injections, it is your responsibility to follow up with your healthcare provider, pharmacy, etc. for subsequent doses according to the appropriate schedule.
• After your exam, your results will be sent to the email address that you used for registration.
• Registration is required because they will need your basic information to provide you the results. However, the registration window will not close until the event that day is closed meaning you can register and participate on the same day.
• You can earn 200 points through our Wellness Program by getting your biometric screening!
