Diabetes Management Tools

Print
Type of Pilot Project:
Public Service
Describe Project and Impact:
Keeping customers up to date on diabetes management tools
Project Description and Expected Outcomes:
The diabesity of our communities is on an upward climb. Most diabetics do not understand the whole picture regarding the possibility of managing this lifestyle change. This would be an ongoing educational seminar/workshop for all who want to keep updated and informed about the reality of diabetes. A forum for diabetics to exchange their struggles and successful stories with others. This would be a monthly ongoing program with a partnership with the local hospital that services this area. RNs are the ones who facilitate these classes.
Why should we select this project?:
Diabetes is an increasing health problem in our society today. Most folks don't read about this disease once they are diagnosed by their doctors. This would also help connect these customers with the many resources in books and videos that deal with this topic. This is not to replace their doctor but to keep the public informed about this life style management issue.
Constraints, Limitations, or Dependencies:
Unable to partner with local hospital to provide the RN facilitator on a monthly basis.
Source of Inspiration:

There are hospitals in the OKC Metro area that provide nutrition classes and diabetes management through food and exercise, but I feel the libraries in the communities we serve will feel less anxiety in attending this program if it is provided by their local libraries.

Partnership suggestion
This project may benefit from pairing with the Nightscout Project organization, a community of diabetics (and their partners/parents/caregivers) using open-source software to improve monitoring of CGM data. The parent organization is available at http://www.nightscout.info/, and the map p\on their page links to several people who are currently using this technology. Perhaps several of them could be vetted for a program or panel discussion?
