In Color

Since we are making photo copies of the applications, is it ok if we use colored paper?

May I suggest a second line for the address? Like a place to put city and state? We did some sample ones to have some practice ones, and the natural thing was to write the street address and then run out of room for the rest on that line.

Should the form include the policy that *two* forms of ID are required for an adult card?

Since the form is for adults as well as students and there are different requirements to prove eligibility for them, the information on the form is includes examples of acceptable forms of ID rather than the stating policy.

Few Questions

Hello,

  1. To piggy back on Amy's comment, should put back the phrase, "parents can use their child's card to check out items for that child"?
  2. I know we probably covered this in training, but why does the customer need to note which library they will pick up their reserves from again?
  3. Since the applications are double-sided, any recommendations on recycling the huge stack we received yesterday?

  1. Each of the versions of the form states "The signing parent/guardian may use his/her child’s card to borrow materials and review current borrowing record for that child."
  2. The default 'branch' in Carl X is the library where the customer obtain his/her library card. This default location is what is used by Carl X in the OPAC for holdings information and the default for reserve/holds pickup. Only staff my change that default for the customer.  Note: The last activity branch was set as the default for all customers in the conversion process so you'll see CNV (Conversion) as the user alias.  For all cards issued with the new system the default location is where they get the card.  So, if the customer wants a different default for reserve/holds pickup only staff may change that, which is why the question about holds is on the LC application form.
  3. Many of our libraries have recycling bins you may use. Or, there may be other uses you'd prefer for the paper.

Card numbers on Application

On the old forms we had a place where we wrote the Library card number new and old. Where on the new application do we write this information? The new app has a place for Account ID type, Statistical Type, Annual Fee Purchaser Account ID #, etc.

On the first version of the form the LC number field was inadvertently omitted; however, all the updated versions have space for the Account/LC #.

Old LC number

The "First card issue date" in VCirc transferred as "date registered" unless the customer obtained a new LC number. If they were issued a new LC number, then the "Current card of date issued" in VCirc transferred as the "date registered". The old LC number is not used and that is why we do not ask for the old card number on the LC application form.

Large Print

We've had several customers comment that the type is too small to read or it is too hard to fill out because everything seems small. Could there be a front and back "large print" version for people who have vision issues?

Version on the form

Is there a way to add the version # to the form? With several people printing forms, sometimes it difficult to determine which form is the most current. Just a thought...

No, nothing is needed. Those were check boxes and we removed them since we do not believe we need that information. "Application for Library Card Account or UPDATE Account" is intended to indicate the form has a dual purpose and we hope this will encourage customers to keep their accounts updated.

My Suggestion

The line that asks "Want to get same day notifications of holds via text?" should be moved below where it asks for their phone numbers. When most customers see that question they will not leave us a phone number if they do not want the text notifications. I know we do not require a phone number from the customers but it certainly helps out circulation should we ever have to contact the customer about returning a case that is missing the dvd or what have you. I believe they would more apt to give us a phone number before they saw that question.

That question was on the same line as the cell phone number request in the previous version of this form. The situation that was happening with it that way was that when a customer checked 'yes' to wanting to receive a text s/he seem to not see/respond to the request for the carrier. Some staff indicated that since customers seem to not see the carrier request that it might be useful to have the phone number and carrier questions on the same line or one on top of the other so that are seen as a 2-piece question for those who want to receive texts.

As with each of the modifications we learn a bit more about how individuals flow through it. It may be better the way it was in the previous version and in a few days we'll know more whether to change it back, keep it like this or do something different.

Is there a way we could make the signature line more noticeable? I like the boxes and I like that it makes it clear that we need a phone number regardless of whether the customer wants notices or not, but we've already had a problem with customers not seeing where they need to sign.
