Safety Committee Minutes - May 24, 2023

Update & Discussion on workers’ compensation injuries: 

 

Discussed new claims that have been reported since our last meeting. Based on a recent claim, dog bite awareness and animal de-escalation tactics might be a good candidate for a monthly safety training topic. 

 

New Work Comp Claim/Investigation Form:  

 

The decision was made to transition to the new form effective June 1st. XChange will be notified of the new change at the June 6th XChange meeting. New forms will be added to the Intranet for managers to use when filing claims.   

 

After completing the Consult Care hotline call, managers should fill out the new form. HUM will use the form in conjunction with the report from Consult Care to investigate the accident and identify potential mitigation strategies.  

 

Consult Care:  

 

24/7 nurse hotline offered through CompSource. The expectation is that managers will use the 24/7 nurse hotline when reporting claims except in the case of life-threatening injuries or injuries needing immediate emergency medical treatment. Mandie is having fliers made and laminated for the branches to post in their breakrooms for reference. Managers should also keep a copy of the steps on file for reference.  

 

If a manager is not present, the librarian in charge should be on the call and should fill out the Work Comp Claim/Investigation Form. If the form can’t be filled out before the end of operating hours, HUM should at least get an email with a brief outline of the accident so they can expect the form the next day. 

 

Review and Discuss Completed Quarterly Safety Checklists 

 

 There have been no issues reported with using the new safety checklists at library branches. Jeff Cover and John Rahal will meet to discuss whether the same safety checklist can be rolled out to all the departments as-is. 

 

Steve and Albert from the Maintenance Department are reviewing the new Maintenance Checklist Forms. 

 

Review Incidents/Accidents: 

 

No vehicle accidents have been reported since the one no-fault incident in February. There was one minor incident with a vendor, but vendor did not seek medical treatment. 

 

Inventory on Large Pieces of Furniture:  

 

Frontline staff feedback indicates that checking the tags on some large pieces of furniture requires a great deal of squatting, bending, and even lifting heavy pieces of furniture in order to see the inventory barcodes. Possible safety tips to add to next year’s inventory notifications: 

  • Ensure that there isn’t a more accessible location to place the barcode stickers, such as the leg of a table instead of the underside. The barcodes can be moved if necessary and secured in place with book tape. 

  • Taking the time to relocate barcodes to the same location on similar items can save a great deal of bending and squatting for future inventories. 

  • For unique, stationary, and difficult to reach items such as projectors and security cameras, take a picture and label it with the item’s barcode once it has been confirmed. This can be used to match items to numbers in future years without getting up on a ladder. 

  • Consider aids such as telescoping inspection mirrors to look underneath large items without having to lift them up or get down on the ground. 

 

Issues with handling sharps: 

 

 We are reevaluating the need for the system in light of the relatively low number of sharps discarded by the system. We have found out that Oklahoma allows sharps to be discarded with regular trash, if we use the right kind of self-sealing and puncture-proof sharps container. We should be able to discontinue the use of the pickup service. 

 

Response Plan for How to Handle Drug Use in Libraries 

 

Dennis and Jeff will be working together to put together a report/plan for our employees based on a sample plan used by a county in Arizona. 

 

Fire Drills/Evacuation Plans 

 

HUM and Dennis Letter will be meeting to create best practices for regular fire drills and new guidelines for location-specific evacuation plans. Once those are in place, managers will be instructed on how to update their evacuation plans and schedule regular fire/evacuation drills at each location. 

 

Emergency Preparedness Workshop 

 

Jeff and Dennis attended this workshop on 5/15. They brought back some useful information that will be used in updating the evacuation plans specifically and the system emergency response procedures generally. 

 

Audit/Walkthrough of Service Center 

Rusty from Insurica reviewed the facility and gave feedback on areas to focus on and other items to maintain to stay in compliance.
