Important FOCUS Reminders

FOCUS registrations are due Sunday, September 28 at 5 PM. All staff members must submit a registration. Please indicate ‘yes’ or ‘no’ on the registration form.

We have noticed there are several people who indicated they want a screening but did not schedule a time for one.  If you have already registered but have not scheduled your screening time through Personalized Prevention, please do so by clicking here. For instructions on scheduling, please view the video here.
